CONSENT & RELEASE FORMS are MANDATORY and must be received PRIOR to a team competing in ANY tournament.  Incomplete or missing forms will disqualify a team from the competition, a policy which is strictly adhered to.  Forms that are missing a team name/number are considered incomplete.

**These forms are required to participate in all FLL Tournaments.  No exceptions.**
